Sacramental Record Request Form

Ministry Forms:

  • Room Reservation Form :  To request the use of a church facility, the Room Reservation Form must be filled out and submitted to the parish office prior to scheduling your event.  NO PHONE REQUESTS WILL BE ACCEPTED.
  • Bulletin AnnouncementAll announcements must be submitted by email or in writing to the parish office by Friday a week prior to announcement appearing in the bulletin. Announcement requests by phone will NOT be accepted. Please follow instructions in the back of the Bulletin Announcement Form.
  • Pulpit AnnouncementMass announcements must be submitted by email or in writing to the parish office by Wednesday of the weekend you would like announcement made.  Announcement requests by phone will NOT be accepted.  Please follow instructions on the back of the Pulpit Announcement Form.
  •  Table Request: If your ministry is in need of a table to set up after the weekend Mass, please fill out Table Request Form and email to Janice Putvin at jputvin@stclementpc.org
  • Facility Work Order: Please submit all requests for maintenance or custodial work by email or in writing to emendez@stclementpc.org.      
  • Check request/Payment ReimbursementPlease submit all requests for payments in writing by filling out the “Check Request/Credit Card Expenditure Form” and attaching receipt(s) and/or documentation to the completed form.  All check requests/reimbursements over $100.00 MUST have the pastor’s approval prior to submitting request to the bookkeeper.
  • Request for Youth ParticipationPlease submit all requests to Myriam Reyna at least 1 month prior to event:  mreyna@stclementpc.org
